Job Title: Counsellor II (without certificate)
Duties / Description
- Participate in the development of individual support plans
for each individual.
Continually update and add to support plan throuhout the year as personal
information changes.
- Support each individual according to the established pholosophy
in action and daily schedule, ensuring that each person is treated with
respect and dignity in all of their supports.
- Participate in the development of any behavioural or realted
intervention strategies.
As needed, collect and compile data and prepare summary reports.
- Show initiative in planning, organizing and implementing activites
that are meaningful to the individuals, utilizing generic support systens
where possible.
- Ensure provisions of care provided to individuals in all areas
fo their needs (medical, personal, physical, financial, equiptment, hygienic,
social recreational, emotional, developmental, and psychiatric) are met
on a day to day basis.
- Administer, record and update all prescription and non-prescription
medication as required.
- Transport clients, using personal vehicle if necessary, to
and from activities and appointments as required.
- Serve as a role model with regards to grooming, hygiene, dressing,
speaking and relating to clients.
- Anticipate and identify problems, consider alternatives, consult
with others as needed, seek approval when necessary and make appropriate
decisions based on the best interest of the individuals.
- Communicate with coworkers, supervisor(s), fanfilies/guardian(s)
and other professionals to meet the needs of each individual.
- Work within a team sharing relevant information interacting
when necessary and maintaining a positive and cooperative relationship
with all team members.
- Verbally notify supervisor and or director of any serious occurrences
immediately after the incident occurs, followed by the completion of the
serious occurrence form.
- Record all required information and observations accurately within 24
hours., ensuring information is passed on to Supervisor.
- Responsible for keeping client files up to date, ensuring documents
are filed in the appropriate binders and overflow boxes.
- Ensure all duties assigned are completed in the allotted time
while maintaining quality.
- Attend all meeting required.
- Participate in and maintain valid certificates in all mandatory
training, i.e. First Aid and Non Violent Crisis Intervention.
- Attend courses or seminars that will enhance self improvement
and skill in the field.
- Follow all Policies and Procedures specified by the Association.
- Be knowledgeable and aware of the Association's policy on
Health and Safety; practice and promote said policy at all times.
- Maintain a professional working relationship with Association
staff, farnilies/guardian(s) and outside agencies.
- Work in other locations outside of assigned area for reason
of
a) work overload in another location
b) staff shortages in another location
c) emergency back-up
- Follow through on any other responsibilities and tasks as
requested.
